1. 請上網填寫 GOOGLE表單 並上傳 新版「演講報告繳交表」電子檔(可掃描或照相)：

2. 報告檔名請以學號(例: r12345678) + 演講日期(例: 9/1)
   (例如: r123456780901，可加姓名)，以避免被遺漏！
3. 檔案格式限doc, docx, odt, pdf, png, jpg
4. GOOGLE表單將於11/16 (週二) 23:59關閉
5. 繳交演講報告次數統計結果如連結：https://reurl.cc/WX847D
6. 本學期最後計次場次為12/30之演講
7. 每月至多計 4 次 演講，遲交不計次

Lecture Report Submission for 11/06-11/12 Lectures now is open:

1. Please fill the GOOGLE FORM and upload your “NTU Department of Chemistry Lecture Report”: 
   https://reurl.cc/WXWxnZ

2. You can scan or take a picture of your report then upload it.
3. Filename should be your student ID (r12345678) + lecture date (mm/dd), e.g., r123456780901. Otherwise, your file may be missed.
4. File extensions: doc, docx, odt, pdf, png, jpg.
5. The Google Form will be closed on 11/16 (Tue.) 23:59.
6. The submission records of NTU Department of Chemistry Lecture Report is as the link below:
   https://reurl.cc/WX847D
7. The final lecture that can be counted toward the 2021 Fall semester is on December 30, 2021.
8. a. Ph.D. students: at least 12 lectures per semester (at most 4 lectures per month)
   b. M.S. students: at least 10 lectures per semester (at most 4 lectures per month)
   c. College seniors: at least 3 lectures per semester
9. Only lectures organized by NTU Chemistry Department can be accepted.
   If you choose to attend a lecture organized by other departments, it would be counted 1/3 times.
   All lectures must be related to Chemistry.
