大家好:
上星期四早上11:50時,在B767-1實驗室中間的抽風櫃,封管玻璃反應瓶突然爆開,裡面含有約250 mL的四氫呋喃和水(約1:1),其中反應瓶的玻璃碎裂濺射,造成在抽風櫃另一側的及同側的兩位同學手臂被玻璃割傷,送醫治理後同學已無大礙。經查證,同學們於事發時並未穿著防護衣及配戴護目鏡。由於化學系過去兩年有超過四起學生因未遵守環安衛規定而在意外中受傷之事件,校長責成本校環安衛中心嚴肅看待本系在實驗操作上的安全問題,並要求化學系於後續校長主持之特別會議中提出改善辦法。肇事之實驗室目前已被停止運作,不得進行任何實驗。本校環安衛中心表明,學生安全是最高指導原則,當學校要封閉一間實驗室,學生是否能如期畢業,重要樣品之保存問題,研究計劃之執行之時限,老師的身分或學術地位,均不在考慮之列。環安衛中心進一步指出,按學校已公告之規定,根據化學系過往之紀錄,中心有權以斷水斷電方式封閉整棟積學館。目前的處理方式,是希望化學系可以亡羊補牢。
現在最重要的工作,是協助被封閉的實驗室可以盡快重新運作。在這裡我希望化學系的同仁可以同舟共濟,積極落實學生在任何貼有「毒性及關注化學物質運作場作」的實驗室隨時穿實驗衣及配戴護目鏡的規定。由於下一起的類似事件可能會導致積學館全棟被封,我在這裡授權本系所有專任教授,在任何「毒性及關注化學物質運作場作」或在公共空間發現環安衛違規事件(例如手持沒有置入保護器皿之NMR tubes等),可以即時作出糾正及紀錄違規學生名字,天氣是否炎熱,實驗是否安全,配戴護目鏡會否影響實驗之進行,均不在考慮之列。若有個別實驗室屢勸不聽,置大家共同利益不顧,本系將召開緊急系務會議,討論是否以斷水斷電的方式處理該實驗室。

Dear all,
On last Thursday room B767-1 had a lab accident and two students were injured. The students are doing well after proper treatment. However, because the injured students did not wear lab coat nor put on eye goggle during the accident, the lab has been shut down by NTU-EHS without any time frame. The NTU president will soon organize a meeting and the Chemistry department must provide a plan to address all the issues related to this accident. On the basis of the NTU-EHS regulations and the poor safety track record of our department, actually the whole chemistry building could have been closed by suspending the water and electricity supply.
To facilitate the re-opening of B767-1, we must strictly enforce the EHS regulations for all labs with the warning sign of "Toxic and Concerned Chemicals Handling Workplace". Because the next violation of EHS regulations may lead to the shut down of the whole chemistry building, I hereby authorize all PIs of our department to intervene when they observe any EHS violations in any laboratories or in the public area (e.g. holding NMR tubes without protective container). If any lab refuses to comply with the safety rules (put on lab coat and goggle at all time), an ad hoc department meeting will be called for to discuss whether a suspension of water and electricity supply of the lab is needed.
