各位化學系師長、同仁及同學大家好：
依系務會議決議，本系環安衛守則內容已修訂如下(詳如附檔)，並於2022年12月30日起公告施行。
環安衛小組之老師得在各實驗場所進行不定時檢查。查獲之違規者，必須重新接受一般安全衛生教育訓練三小時(由實驗室借用系上會議室辦理)。第一次違規者本系將停止其門禁卡及使用系上所有共同儀器之權利1個月，另禁止進出所屬學生休息室與實驗場所2星期；第二次違規者將停止使用系上所有共同儀器之權利2個月，另禁止進出所屬學生休息室與實驗場所1個月；第三次(含)違規者將停止前述所有權利6個月並送交系環安衛會議討論。有三次（含）以上違規紀錄者將被取消申請本系各獎項之權利。系主任裁定違規情節重大者，本系得立刻終止其於本系進行實驗或工作之權利直至另行通知。
停權期間，若該成員被發現仍在所屬學生休息室或實驗場所出沒，系主任得召開臨時系務會議，經出席人員三份之二同意，系上將按會議所決定的時期逕行停止該實驗場所之運作。
被停權學生，經指導教授同意，得以書面向理學院院長申訴。
The EH&S team will conduct inspections at each laboratory from time to time. If a violation is found, the violator will be required to retake a 3-hour general EH&S training course in a departmental meeting room (organized by the research group). For the first time offender, his/her access card and the right to use all public equipment in the department will be suspended for one month, and access to the student's office and laboratory will be banned for two weeks. For the second violation, the violator will be banned from using all public equipment in the department for two months, plus a one-month ban on access to student’s office and laboratory. As to the third time (inclusive), all the aforementioned rights will be suspended for six months and the case will be submitted to the EH&S meeting for discussion.
Anyone with three or more violations will be disqualified from applying for the department's awards. If the department chair judges that the violation is a serious one, all the aforementioned rights of the violator will be terminated immediately until further notice.
During the suspension period, if the violator is found in the student’s office or laboratory, the department chair may call for an ad hoc department meeting. With the approval of 2/3 of the attendants, all the operations of the laboratory will be suspended for the prescribed period.
A person whose right to work in a laboratory has been suspended may, with the consent of the supervisor, file a written appeal to the Dean of the College of Science.
