化學系各位成員大家好：
5/2清晨，A416實驗室發生淹水，大量的水滲入A416的管道間再漏到下方樓層，並造成A217教室漏水。
經與該實驗室人員座談，事發經過整理如下：
1.5/1晚間開始進行實驗的冷卻水出水量過大，在不確定之時間點，出水管脫離水槽。
2.大量的水沿排煙櫃檯面溢流至地面，導致A416地面積水，滲入管道間後再漏到二樓，造成A217漏水。
缺失及建議說明如下：
1.該實驗室已將出水管固定，並於水龍頭上標示冷卻水出水量。
2.實驗室為一小群體，如同化學系為一大群體，一損俱損，一榮俱榮；成員間應相互提醒。
由於此類人為疏失事件已發生數次，除再次於全系教育訓練宣導外，將於系務會議討論後續因應。

Dear all,
On the morning of May 2, flooding occurred in lab A416.
A large amount of water seeped into the piping space of A416 and subsequently leaked to the lower floor, causing water leakage in Classroom A217.
Based on discussions with the lab personnel, the sequence of events has been summarized as follows:
- On the evening of May 1, an experiment was initiated, during which the cooling water outflow was excessive. At an unknown point in time, the outlet pipe became detached from the water tank.
- A large volume of water overflowed along the fume hood surface onto the floor, resulting in water accumulation in A416. The water then seeped into the piping space and leaked to the second floor, leading to water leakage in A217.
Identified issues and recommendations are as follows:
- The lab has secured the outlet pipe and marked the cooling water flow rate on the faucet.
- The lab is a small group, just as the Department of Chemistry is a larger one. A loss for one is a loss for all; a success for one is a success for all. Members should remind and support each other.
Since similar human error incidents have occurred for several times, in addition to reinforcing awareness during departmental training, further responses will be discussed in the upcoming departmental meeting.
